Start with the notice
Identify the penalty, period, amount and response instructions. Then establish what should have happened, what actually happened, and when the problem was corrected.
Keep the facts separate from assumptions about why the IRS assessed the charge. An incorrect account record may require correction, while a valid assessment may require a relief request.
Build a chronological explanation
Describe the circumstances that prevented compliance, their start and end dates, and the steps taken to address the tax obligation afterward. Explain the connection between the circumstances and the specific filing, payment or deposit requirement.
Avoid adding events or diagnoses that are not supported by the records. If an exact date is unknown, say so instead of presenting an estimate as a confirmed fact.
Match evidence to the claim
